Physical death is where our body dies and our spirit lives on. Salvation from physical death is the resurrection. The resurrection is where the bodied and spirit are reunited to never be separated again. The bodied will no longer be subject to illness. Resurrection is a free gift to all mankind.
Spiritual death is being separated man from God. Salvation from spiritual death is repentance. Through Jesus' atonement are given the ability to repent and return to live with God. Repentance is a free gift, but we must use it.
Below I have listed stuff on being saved some are about salvation from physical death. Some are about salvation from spiritual death. Some I have not categorized yet.
Revelation 1:18 speaks of the two types of death.
he that liveth, and was dead; and, behold, I am alive for evermore, Amen; and have the keys of hell and of death.
Saved from Physical Death1 Corinthians 15:22
22 For as in Adam all die, even so in Christ shall all be made alive.
28 Marvel not at this: for the hour is coming, in the which all that are in the graves shall hear his voice, 29 And shall come forth; they that have done good, unto the resurrection of life; and they that have done evil, unto the resurrection of damnation.
6 And all flesh shall see the salvation of God.
Saved from Spiritual DeathHebrews 5:9
And being made perfect, he became the author of eternal salvation unto all them that obey him;
21 ¶ Not every one that saith unto me, Lord, Lord, shall enter into the kingdom of heaven; but he that doeth the will of my Father which is in heaven.
22 Many will say to me in that day, Lord, Lord, have we not prophesied in thy name? and in thy name have cast out devils? and in thy name done many wonderful works?
23 And then will I profess unto them, I never knew you: depart from me, ye that work iniquity.
24 Therefore whosoever heareth these sayings of mine, and doeth them, I will liken him unto a wise man, which built his house upon a rock:
25 And the rain descended, and the floods came, and the winds blew, and beat upon that house; and it fell not: for it was founded upon a rock.
26 And every one that heareth these sayings of mine, and doeth them not, shall be likened unto a foolish man, which built his house upon the sand:

If we hear his words, but do not follow them, then we fall. His words are the will of the Father and if we do not do the will of the Father, we do not enter the kingdom of heaven.
20 For I say unto you, That except your righteousness shall exceed the righteousness of the scribes and Pharisees, ye shall in no case enter into the kingdom of heaven.
Righteousness (repenting of and forsaking our sins) allows you entrance into the kingdom of heaven.
10 If ye keep my commandments, ye shall abide in my love; even as I have kept my Father’s commandments, and abide in his love.
4 And I prayed unto the Lord my God, and made my confession, and said, O Lord, the great and dreadful God, keeping the covenant and mercy to them that love him, and to them that keep his commandments;
1 John 1:6-7
6 If we say that we have fellowship with him, and walk in darkness, we lie, and do not the truth:
7 But if we walk in the light, as he is in the light, we have fellowship one with another, and the blood of Jesus Christ his Son cleanseth us from all sin

“Because of what He accomplished by His atoning sacrifice, Jesus Christ has the power to prescribe the conditions we must fulfill to qualify for the blessings of His Atonement. That is why we have commandments and ordinances. That is why we make covenants. That is how we qualify for the promised blessings. They all come through the mercy and grace of the Holy One of Israel, “after all we can do” (2 Nephi 25:23).”
30 Therefore I will judge you, O house of Israel, every one according to his ways, saith the Lord God. Repent, and turn yourselves from all your transgressions; so iniquity shall not be your ruin.
31 Cast away from you all your transgressions, whereby ye have transgressed; and make you a new heart and a new spirit: for why will ye die, O house of Israel?
32 For I have no pleasure in the death of him that dieth, saith the Lord God: wherefore turn yourselves, and live ye.
If you do not cast away your transgressions then you will die spiritually.

My Thoughts on Revelation 21:27
1 Thessalonians 4:3-4
3 For this is the will of God, even your sanctification, that ye should abstain from fornication:
4 That every one of you should know how to possess his vessel in sanctification and honour; These verses speak of being sanctified (saved) when abstaining from fornication and being honorable.
2 Nephi 28:8
8 And there shall also be many which shall say: Eat, drink, and be merry; nevertheless, fear God—he will justify in committing a little sin; yea, lie a little, take the advantage of one because of his words, dig a pit for thy neighbor; there is no harm in this; and do all these things, for tomorrow we die; and if it so be that we are guilty, God will beat us with a few stripes, and at last we shall be saved in the kingdom of God.
11 And that, knowing the time, that now it is high time to awake out of sleep: for now is our salvation nearer than when we believed. In this chapter Paul detailed how being righteous leads to salvation. He states here that they are closer now to salvation than when they believed. Why are they closer? Because they have obeyed the Lord's commandments. That brings them closer to salvation than simply believing.
1 Corinthians 15:1-2
1 Moreover, brethren, I declare unto you the gospel which I preached unto you, which also ye have received, and wherein ye stand;
2 By which also ye are saved, if ye keep in memory what I preached unto you, unless ye have believed in vain. Keeping in memory would appear to be more than just believing.
31 Then said Jesus to those Jews which believed on him, If ye continue in my word, then are ye my disciples indeed;
32 And ye shall know the truth, and the truth shall make you free. You shall know truth if you are Christ's disciples. If you know truth, ye are free. Not just because you believe. Only if you continue in his word.
1 John 1:7
7 But if we walk in the light, as he is in the light, we have fellowship one with another, and the blood of Jesus Christ his Son cleanseth us from all sin. If you walk as Jesus walked, then his blood cleanses us from all sins that we may have previously committed.
2 Nephi 31:19-21
19 And now, my beloved brethren, after ye have gotten into this strait and narrow path, I would ask if all is done? Behold, I say unto you, Nay; for ye have not come thus far save it were by the word of Christ with unshaken faith in him, relying wholly upon the merits of him who is mighty to save.
20 Wherefore, ye must press forward with a steadfastness in Christ, having a perfect brightness of hope, and a love of God and of all men. Wherefore, if ye shall press forward, feasting upon the word of Christ, and endure to the end, behold, thus saith the Father: Ye shall have eternal life.
21 And now, behold, my beloved brethren, this is the way; and there is none other way nor name given under heaven whereby man can be saved in the kingdom of God. And now, behold, this is the doctrine of Christ, and the only and true doctrine of the Father, and of the Son, and of the Holy Ghost, which is one God, without end. Amen.
37 Judge not, and ye shall not be judged: condemn not, and ye shall not be condemned: forgive, and ye shall be forgiven:
You have to forgive to be forgiven. You have to not judge in order to not be judged. If you are judged and not forgiven, you are not saved.
11 For behold, and also his blood atoneth for the sins of those who have fallen by the transgression of Adam, who have died not knowing the will of God concerning them, or who have ignorantly sinned.
12 But wo, wo unto him who knoweth that he rebelleth against God! For salvation cometh to none such except it be through repentance and faith on the Lord Jesus Christ.
3 Without natural affection, trucebreakers, false accusers,incontinent, fierce, despisers of those that are good,
4 Traitors, heady, highminded, lovers of pleasures more than lovers of God;
5 Having a form of godliness, but denying the power thereof: from such turn away.
8 Bring forth therefore fruits meet for repentance:

Doctrine and Covenants 59:21
21 And in nothing doth man offend God, or against none is his wrath kindled, save those who confess not his hand in all things, and obey not his commandments.
